				<div class="et_pb_toggle_content clearfix"><p style="text-align: justify;">In Cambodia today we can still see the ongoing effect of the war that embroiled the country until the 1990&#8217;s. Despite efforts to improve the public healthcare system, and efforts to introduce a universal social security system, the healthcare system in Cambodia still retains many structural problems which affect its quality of service.</p>
<p style="text-align: justify;">The National Social Security Fund (NSSF), was introduced in 2008, its aim was to provide basic social security to workers in the private sector and their dependants, however the cover is limited to work related accidents. The pension system is only available to civil servants. The introduction of a public healthcare system was planned for 2012, however it has still not been applied. To this day, the NSSF covers only 10% of the working population.</p>
<p style="text-align: justify;">In the absence of an efficient and reliable healthcare system, it is a private health insurance policy is indispensable in Cambodia.</p>
<p style="text-align: justify;">In regards to the quality of care, the public hospitals are generally underequipped and greatly below the standards of Western hospitals.</p>
<p style="text-align: justify;">The private hospitals offer a better quality of care but do not necessarily have the specialists or the equipment on-site to treat certain medical conditions. In the event of a complicated medical condition, it is recommended to seek treatment outside of Cambodia (e.g. Bangkok in <a href="https://www.expatassure.com/thailand/" target="_blank" rel="noopener noreferrer">Thailand,</a> Ho Chi Minh in <a href="https://www.expatassure.com/vietnam/" target="_blank" rel="noopener noreferrer">Vietnam</a> or in <a href="https://www.expatassure.com/china/" target="_blank" rel="noopener noreferrer">China</a> or in <a href="https://www.expatassure.com/singapore/" target="_blank" rel="noopener noreferrer">Singapore),</a> or even request repatriation to your country of origin. Even though healthcare costs are very low in Cambodia, the services offered can be very limited depending on where you are. This increases the possibility of needing a medical evacuation which could lead to an extremely high medical bill.</p>
<p style="text-align: justify;">For these different reasons it is indispensable to have private health insurance in Cambodia including evacuation and/or repatriation, so that costs related to an evacuation as well as medical costs from outside of Cambodia are covered by your policy. Please be aware that your health insurance policy will only cover an evacuation/repatriation in the case of emergencies. <p style="text-align: justify;"><strong>Consultation with a GP or specialist:</strong> the cost can vary greatly. Doctors can charge between $5 and $100 (£4 -£78) or even more depending their surgery. As a general rule, consultation costs tend to be lower in clinics, however are often accompanied by many tests (x-rays, scans etc.)</p>
<p style="text-align: justify;">Please see below for a general idea of costs in doctors’ surgeries in Phnom Penh as recommended by the French Embassy, a consultation with a GP or specialist would cost $35 (£27.5). In the Khema Clinic, the cost of a consultation would be $25 (£20). At the CT Polyclinic in Sihanoukville the cost would be $50 (£39). At the International SOS clinic, a GP consultation would be around $61 to $88 (£48 &#8211; £69), depending on the treating doctor.</p>
<p style="text-align: justify;"><strong>Consultation with a dentist:</strong> at the European dental clinic in Phnom Penh, the cost of a dental consultation is $25 (£20). A porcelain crown on a metallic base would cost around $470 (£370) and an entirely porcelain crown would cost $650 (£512). At the International Dental Clinic in Phnom Penh, a consultation costs between $15 and $25 (£12/£20), the cost of a crown is around $150 to $350 (£118-£275).</p>
<p style="text-align: justify;"><strong>Cost of an overnight hospital stay in an individual room: </strong>In a private Cambodian hospital an this would cost in the region of $120 &#8211; $150 (£94 &#8211; £118). International hospitals will be more expensive, for example the International SOS medical clinic, an overnight stay in a three-bed room will cost in the region of $650 (£527). 				<div class="et_pb_toggle_content clearfix"><p style="margin: 0cm; margin-bottom: .0001pt; text-align: justify; vertical-align: baseline;"><span style="font-size: 10.5pt; font-family: 'Helvetica',sans-serif; color: #666666;">Vietnam’s economic development over the past decade has been accompanied by an improvement in public health, and government expenditure going into the Vietnamese health service is increasing. Moreover, Vietnam has recently implemented a reform to provide universal healthcare coverage to the entire population. There are two types of health cover in Vietnam, managed by the Vietnam Social Security (VSS):</span></p>
<ul>
<li style="text-align: justify;">Compulsory social security cover: it applies to all workers on permanent contracts of more than three months. It is also applies to several other categories, with the aim of making it universal (children, poor people, students, those over 90 years of age, war veterans…)</li>
<li style="text-align: justify;">Voluntary health insurance: this applies to people who are not covered by the mandatory scheme: the self-employed and informal workers.</li>
</ul>
<p style="text-align: justify;">Expatriates must contribute to the compulsory health insurance, but as will be explained in the following paragraph, this is often not enough and many expatriates choose to subscribe to a private medical insurance in Vietnam.</p>
<p><strong>The quality of the Vietnamese healthcare system</strong></p>
<p style="text-align: justify;">Despite numerous efforts to provide healthcare cover to the entire population, the Vietnamese health system remains largely underfunded and suffers from structural inefficiencies.</p>
<p style="text-align: justify;">The level of cover offered by the public system is low, and the rest of the costs left to those using the services remain high, even though the highest users are found among the poor. For example, public hospitals apply a fee for some complicated medical procedures.</p>
<p style="text-align: justify;">The quality of care in public hospitals is still far from being up to the standards of western countries. Vietnam’s hospitals are overloaded. According to the French Embassy in Hanoi, it is not uncommon for hospitals to have an occupation rate of 200%, particularly those which are centrally located. Meanwhile, there is not enough medical equipment or medical staff to treat everyone. These structural problems seriously affect the general health services in hospitals.</p>
<p style="text-align: justify;"><strong style="line-height: 1.5;">Inequalities between urban and rural areas</strong></p>
<p style="text-align: justify;">It is also important to note that the quality and availability of medical services varies considerably between urban and rural areas. Public hospitals in the rural provinces are the most underfunded and underequipped. Many people living in the provinces therefore travel into major urban areas for their medical treatment, which serves to prolong the waiting lists in hospitals in big cities such as Hanoi or Ho Chi Minh City.</p>
<p style="text-align: justify;">Because of these structural problems in the public sector, it is recommended to subscribe to a private health insurance in Vietnam. Private hospitals also offer services that better meet the expectations of the expatriate population. In general these hospitals have an international medical staff, who speak English. The charges in international clinics are higher. For example, in the SOS Clinic in Hanoi, consultation costs 2.9million VND (£85/114€).</p>

<p style="text-align: justify;">The most important reform was the introduction of a national healthcare system, made compulsory in 1996. It offers healthcare to Russian citizens, as well permanent and temporary residents.</p>
<p style="text-align: justify;">This system covers: primary care, preventative care, emergencies (except for specialised emergencies such as air ambulance), and specialised care in all public medical establishments. However, some services are not offered, such as dental, prosthetics, rehabilitation and convalescence, as well as prescribed medicines. In order for these to be covered you would need to sign up to a private health insurance.</p>
<p style="text-align: justify;"><strong>The structure of health services</strong></p>
<p style="text-align: justify;">In Russia, there are no private surgeries and all practitioners see their patients in either a clinic or a hospital.</p>
<p style="text-align: justify;">The administration of the health services is divided into different levels: federal, regional, and municipal.</p>
<ul>
<li style="text-align: justify;"><strong>Health posts:</strong> run at a municipal level, they offer out-patient treatment: routine check-ups, vaccinations, and the treatment of non-serious accidents.</li>
<li style="text-align: justify;"><strong>Health centres:</strong> with teams of nurses, gynaecologists, paediatricians, and mid-wives; these establishments are usually specialised in maternity and also basic routine surgery. They are also usually run by the local municipality.</li>
<li style="text-align: justify;"><strong>Urban polyclinics: </strong>these are regional establishments, offering a wide-range of services: screening, treatment of chronic conditions, follow-up care. Depending on their size they may even have on-site specialists (cardiologists, oncologists, obstetricians etc).</li>
<li style="text-align: justify;"><strong>Specialised medical hospitals:</strong> usually found in large cities, these establishments offer all medical services and are open 24 hours a day. Federal hospitals represent 4% of the number of beds available within the public sector.</li>

<p style="text-align: justify;"><strong>Quality of healthcare</strong></p>
<p style="text-align: justify;">The Russian healthcare system has seen some difficulties, in particular linked to a lack of funds. Today, healthcare in Russia remains below the standard of other industrialised countries: there is a lack of medical equipment, differing standards of care depending on the medical establishment, shortfalls in primary care etc. The well-reputed hospitals in Moscow or Saint Petersburg do not offer all services (e.g. contagious illness and maternity complications are not treated).</p>
<p style="text-align: justify;">In the public sector, English speaking doctors are few and far between. In order to see an English speak doctor it is advisable to seek treatment at a private clinic, however fees in the private sector can be high.</p>

				<div class="et_pb_toggle_content clearfix"><p style="text-align: justify;"><strong>Public healthcare</strong></p>
<p style="text-align: justify;">If you are a permanent resident in Thailand, you must subscribe to the compulsory national insurance system on the same terms as Thai nationals. Healthcare is free within the limits of the facilities in the area where the patient lives, on presentation of their universal health insurance card (gold card).</p>
<p style="text-align: justify;">If you are not eligible for the national healthcare system or if you want to be treated in the medical facility of your choice, you will have to pay for treatment within the public sector; you would have to pay independently and then you may be reimbursed by your insurance providers if you are covered.</p>
<p style="text-align: justify;">There is a good level of care in the Thai public sector and that is reflected in the well-being of the population: among developing countries Thailand has one of the highest life expectancy rates. However, there are disparities between medical facilities in cities and rural areas. In the event of an accident, you will be taken to the nearest hospital, and should you need to be transferred to a different hospital for medical reasons this would be your own responsibility and paid out of your own pocket.  For this reason, an insurance policy including an evacuation cover is strongly recommended.</p>
<p style="text-align: justify;">An evacuation cover takes you to the nearest centre of medical excellence for treatment but will not repatriate you once you have stabilised. Adding repatriation to your evacuation benefits would cover the costs to have you sent back to your country of residence, or country of nationality, once your condition has stabilised. (Please note that repatriation will always cover evacuation, but an evacuation only cover will not include repatriation).</p>
<p style="text-align: justify;">Healthcare in Thailand is very “hospital-centric”. Whether you are in the private or the public sector, consultations with GPs and specialists tend to be carried out in hospitals. Doctor’s surgeries, like in the UK, are very rare. Many hospitals even have their own dental surgeries. Consequently, public hospitals are often more crowded than private hospitals.</p>
<p><strong>Private healthcare</strong></p>
<p style="text-align: justify;">Private hospitals in Thailand have a high quality of service, with up-to-date and ultra-modern medical equipment and technology. The staff are very efficient, and waiting times for consultations, treatments, and operations are very short.</p>
<p style="text-align: justify;">The expatriate community tends to opt for the private sector as they are still able to get good value for money. This allows them to choose the medical centre where they wish to be treated, and many private medical centres also have international staff who speak English.</p>
<p style="text-align: justify;">A number of the hospitals are listed on the stock exchange and are internationally recognised. Something that often surprises expatriates is the fact that so many medical centres are so modern that they seem more like luxury hotels than hospitals!</p>
<p style="text-align: justify;">Alongside this is the fact that their prices are relatively low compared to European hospitals. For these reasons, Thailand is a popular destination for medical tourism. However, « relatively low » doesn’t mean that healthcare is cheap and getting treated can still be extremely expensive, especially for a hospitalisation. Moreover, many services require an advance payment which can be very costly. It is therefore important to have private insurance covering hospitalisation as a minimum.</p></div>

<p style="text-align: justify;">If you would prefer to be treated in your country of nationality or a different country, your medical costs would be reimbursed (within the limits of the zone of cover of your chosen policy). Moreover, when you leave Thailand and decide to move to another country your expatriate health insurance may be able to follow you.</p>
<p style="text-align: justify;">As mentioned above, it is recommended to take out insurance covering at least hospitalisation and evacuation.</p>
<p style="text-align: justify;">If you are a permanent resident in Thailand, subscribing to an international health insurance does not exempt you from signing up to the local health care system; in this case your international health insurance would top-up your state healthcare.</p>
<p>Please note that expat health insurance plans are more expensive than local insurance plans. For people over 60 years, private health insurance becomes extremely expensive and international insurers don&#8217;t accept new clients over 70 years old (for some insurers, the age limit for the application is 65 years old). </p>

<p style="text-align: justify;">The public healthcare system in Hong Kong is financed by the State, without the need for a contribution from the employer or the employee and the fees for doctors and hospitals are subsidised. Overall, the quality of healthcare in the public and the private sector are similar.</p>
<p style="text-align: justify;">The main difference is the level of service received. The public healthcare system is of good quality, attracting patients from all over China and neighbouring countries. Consequently, waiting lists can be lengthy (not including A&amp;E) for certain treatments or for in-patient treatment.</p>
<p style="text-align: justify;">In semi-private and private medical centres, waiting lists are much shorter, the rooms are more comfortable and the visiting hours are more flexible. These medical centres charge among the highest fees in the world, for which it is essential to have subscribed to a medical insurance.</p>
<p>The public healthcare system is only accessible to those who hold a local identity card, subject to certain residency conditions. If you do not hold a local identity card you are not able to benefit from the subsidised rates and you must pay the entirety of your medical costs. In this case, it is essential to take out local or international private medical insurance.</p>

<p style="text-align: justify;">Local health insurance policies are based on the average medical cost of an individual, the premiums are then adjusted depending on the amount of claims you make. 				<div class="et_pb_toggle_content clearfix"><p style="text-align: justify;">Singapore’s health system is the shining star of the small Republic, which remains the envy of many other governments. Health spending in Singapore is moderate (4.6% of GDP against 9.3% in the UK), while health indicators are among the highest in the world: life expectancy is 82.5 years on average, against 80.5 in the UK.  Infant mortality is 3 per thousand births (it is 5 per thousand in the UK).</p>
<p style="text-align: justify;">What is Singapore’s secret for such an efficient health system? The healthcare system is funded in a way that increases awareness of the cost of healthcare among the population. While in many countries citizens are disconnected from the cost of their health, in Singapore each person saves a certain amount of their salary every month to participate in financing the medical expenses that they will encounter during their lifetime. The unused savings in the account generate interest. The aim of this system is to avoid overuse of medical care / treatment.</p>
<p style="text-align: justify;">Meanwhile, due to the quality of their doctors, state of the art equipment, and quality of service offered, Singapore has become the centre of excellence for healthcare in Asia, whether in the private sector or the public sector. Treatment for cancer is very highly rated. Singapore has 20 health facilities internationally accredited by the JCI (Joint Commission International), an American institution establishing a global quality standard for medical facilities, and the Gleneagles Hospital is one of the top ten best hospitals in the world.</p>
<p> <span style="color: #000000;">How does the health care system work</span></p>
<p>The state of Singapore covers the majority of the healthcare costs of its citizens in the form of subsidies within public health institutions, often covering up to 80% of costs. The Singaporean health system is organized around the &#8216;3M&#8217;:  Medisave, MediShield and Medifund. It also allows for flexibility in the choice between the private and the public sector.</p>
<p>If you are a foreigner with permanent residency, subscription to the Singaporean healthcare system is compulsory.</p>
<h4><span style="font-size: medium;">Medisave</span></h4>
<p>Medisave is the universal medical savings scheme, enabling every citizen to participate in their individual / family’s future health costs. Employees and their employers save a percentage of their salary, around 7 to 9.5 % of the employee’s monthly salary, in an exclusive savings account. The money is transferred to an organisation called the Central Provident Fund (CPF) and the funds are then used to contribute to the health care costs of the accountholder and his family (spouse, children, and parents).</p>
<p>People have the freedom to use their Medisave savings for treatment in the private sector but they do not benefit from subsidised rates.</p>
<h4><span style="font-size: medium;">Medishield</span></h4>
<p>Medishield is an optional health insurance cover. The contributions can be deducted from the Medisave savings account. Medishield aims to cover serious illness or more expensive forms of care (intensive care, surgery, transplants&#8230;) which Medisave is not sufficient for.</p>
<h4><span style="font-size: medium;">Medifund</span></h4>
<p>Medifund caters for the poorest. This is a fund created by the state in 1993 with a capital sum of 200 million Singapore dollars. The state uses the interest on the capital (S$3bn in 2012) to pay for health expenses of people who cannot afford Medisave savings or who have difficulty meeting medical expenses.</p>
<h4><span style="font-size: medium;">Private health insurance approved by Medisave</span></h4>
<p>It is also possible to purchase private health insurance from a list of approved Medisave (<a href="http://www.moh.gov.sg/content/moh_web/home/costs_and_financing/schemes_subsidies/Medishield/Medisave-approved_Insurance.html" target="_blank" rel="noopener noreferrer">Medisave -approved Integrated Shield Plan</a>) insurance companies.  This coverage can be bought in addition to Medishield (it is not possible to subscribe without having Medishield). This allows the policyholder an additional private insurance to seek treatment in private hospitals or to pay for more comfortable rooms in the public sector.</p></div>
