<p style="text-align: justify;">Despite the NHS operating in all four countries of the UK (England, Scotland, Wales and Northern Ireland), it is managed differently in each, and therefore  NHS Scotland has some different rules. For example, prescription drugs are completely free, and  eye tests and dental check-ups are also free for the entire population. Scotland is the only one out of the four countries to offer domestic personal care and nursing services for over 65s.</p>
<p style="text-align: justify;">The NHS in Scotland has the reputation of being the most efficient compared to its neighbours, and Scotland has <a href="https://www.nuffieldtrust.org.uk/news-item/scotland-s-nhs-has-lessons-for-the-rest-of-the-uk" target="_blank" rel="noopener noreferrer">been used as an example to be followed</a> by other NHS organisations.</p>
<p style="text-align: justify;"><strong>What does NHS Scotland cover?</strong></p>
<p style="text-align: justify;">As in the rest of the UK, access to free healthcare is a fundamental principle of the NHS in Scotland. Free services for all categories of the population via NHS Scotland are:</p>
<ul style="text-align: justify;">
<li>Appointments with general practitioners and specialists</li>
<li>Stays and care received in hospitals and clinics</li>
<li>Accident and emergency services</li>
<li>Prescription drugs</li>
<li>Eye tests and dental examinations</li>
</ul>

<p style="text-align: justify;">The paid NHS services are free for certain categories of the population – these include pregnant women, children, full-time students, people over 60, people with low resources or with specific medical conditions. They may also receive vouchers towards optical costs. Travel costs for hospital appointments are subsidised for people with low resources.</p>
<p style="text-align: justify;"><strong>The limitations of Scotland&#8217;s NHS</strong></p>
<p style="text-align: justify;">Although the NHS in Scotland is seen as an example to follow for the rest of the UK, it is under great financial pressure, as with everywhere in the UK, which can affect the quality and performance of the public health service. One of the most common problems for patients is long waiting times to see a specialist or to receive certain treatments.</p>
<p style="text-align: justify;">More generally, the functioning of the NHS is not always suitable for the expat population. With NHS Scotland:</p>
<ul style="text-align: justify;">
<li>Patients have the choice of which doctors’ surgery to register with (within the geographical/catchment area it covers), but not necessarily their choice of general practitioner.</li>
<li>The consultation time with the general practitioner is considered too short (the average duration is ten minutes) and in general only one medical problem is treated per consultation.</li>
<li>It is necessary to go through a general practitioner to get referrals to specialists, including gynaecologists or paediatricians. You cannot choose the specialist/consultant. You will usually be treated by the doctor on-duty at the time of your appointment.</li>
<li>Shared wards in public hospitals lack privacy.</li>
</ul>
<p style="text-align: justify;">For these reasons, many expats in Scotland prefer to top up their NHS coverage with UK or international private health insurance.</p></div>

<p>Having a PMI makes it possible to consult a private medical specialist and to access tests or treatment very quickly in the private sector, therefore avoiding the long waiting periods in the public sector (although you still need to be referred by a GP first). A PMI also allows you to be hospitalised in private facilities where the stay will be more comfortable.</p>
<p>This type of insurance, which is often cheaper than expat health insurance, may be suitable for those who are generally satisfied with the NHS but wish to avoid its more major inconveniences, while agreeing to receive treatment only in the United Kingdom.</p>
<p>Many UK companies provide PMI-type health insurance to their employees. However for many expats in Scotland, local health insurance does not always meet their needs:</p>
<ul>
<li>This type of health insurance does not cover consultations of private GPs, maternity, dental and optical.</li>
<li>In order to consult a private medical specialist (including a paediatrician or a gynaecologist), you must first go through a GP.</li>
<li>Dental and optical costs are not covered. These options can be added separately but reimbursement limits are low.</li>
<li>The monitoring of chronic conditions is not covered (an exception is often made for cancer).</li>
<li>This type of insurance only covers you in the UK.</li>
<li>You may only consult specialist physicians who are members of your insurance company&#8217;s network. Therefore, as an expat, if you wish to consult a doctor of your nationality practicing in Scotland, it may be that they are not approved by your insurer and therefore not covered by your policy.</li>

				<div class="et_pb_toggle_content clearfix"><p style="text-align: justify;">Switzerland is amongst one of the biggest spenders per capita in Europe on its residents&#8217; health, and Swiss healthcare is renowned for being very good. As a result, however, Swiss health insurance is expensive for individuals: a source of controversy in the country.</p>
<p style="text-align: justify;">The Swiss health insurance scheme is called <em>l’assurance maladie</em> or LAMal (basic health insurance) in French, and is also known as KGV (<em>Krankenversicherungsgesetz </em>in German). It is governed by the Federal Law on Compulsory Health Care. Cantons in Switzerland each have their own regulations for implementing this law.</p>
<p style="text-align: justify;">The LAMal operates in a semi-public, semi-private way: unlike other countries where health insurance services are managed by a public body, the Swiss basic health insurance is managed by private insurance companies strictly regulated by law and registered by the LAMal.</p>
<p style="text-align: justify;">These insurance companies, known as &#8216;sickness funds&#8217; or &#8216;health insurers&#8217;, are not-for-profit. Unlike conventional private insurance, Swiss health insurance companies must accept any person residing in the region of their activity, regardless of age or state of health.</p>
<p style="text-align: justify;"><strong>Features of the Swiss basic health insurance</strong></p>
<p style="text-align: justify;">Basic health insurance covers routine medical care, prescription drugs, hospital charges in shared rooms, maternity costs and other treatments in certain cases (for example vaccines, home care, health exams and gynaecology).</p>
<p style="text-align: justify;"><strong>Disadvantages of the Swiss basic health insurance</strong></p>
<p style="text-align: justify;">A number of medical procedures such as dental care (excluding serious illnesses or accidents), optical care, physiotherapy and hospital care in private rooms are not reimbursed. Many Swiss people therefore take out additional insurance to cover these types of treatments.</p>
<p style="text-align: justify;"><strong>Your obligations as a foreign resident in Switzerland</strong></p>
<p style="text-align: justify;">If you are residing in Switzerland, you have the obligation to join the LAMal within three months of arriving, and you must choose a basic insurance issued by a health insurance company. For a stay of less than three months, LAMal membership is not mandatory.</p>
<p style="text-align: justify;">Certain categories of foreign residents are not required to join the LAMal as long as they provide proof of equivalent foreign insurance. These include seconded workers and those in a period of training or development (students and trainees). People in certain other situations may be exempt from LAMal membership, but the rules vary according to the different cantons, so you will need to contact your canton of residence to find out more.</p></div>

				<div class="et_pb_toggle_content clearfix"><p style="text-align: justify;">Medical costs in Switzerland are extremely high compared to certain other European countries, to the point where a certain number of Swiss people avoid going to the doctor, or choose to go to France for their treatment, especially in the case of dental care.</p>
<p style="text-align: justify;">If you are affiliated with LAMal, you must pay for your consultations out-of-pocket up to a certain amount before receiving basic health insurance coverage. This deductible is at least 300 Swiss francs (£230/$300) per year, but the amount may be higher depending on the insurance chosen (maximum 2,500 Swiss francs or £1,900/$2,500). Only maternity and children&#8217;s healthcare are not subject to a deductible.</p>
<p style="text-align: justify;">From the moment the medical bills during the year exceed the amount of the deductible, the health insurance covers the following expenses, of which you must still contribute 10%. This share is limited to 700 Swiss francs (£535/$700) per year or 350 francs (£270/$350) per year and per child. In the case of a hospital stay, adults must contribute 15 francs (£11/$15) per day.</p>
<p style="text-align: justify;">If you are a European traveller with a European health insurance card (EHIC), your participation will be 92 francs (£70/$92) for a period of 30 days and 15 francs per day in the case of a hospital stay (for children, only a participation of 33 francs or £25/$33 for a period of 30 days applies).</p>
<p style="text-align: justify;"><strong>Cost of a consultation with a doctor: </strong>The cost varies depending on the canton, but expect to pay around 18 francs (£14/$18) for the first five minutes. The rate then increases in increments of five minutes. A consultation of 20 minutes is therefore around 72 francs (£55/$72).</p>
<p style="text-align: justify;"><strong>Dentist: </strong>A check-up and descaling will cost around 300 francs (£230/$300).</p>
<p style="text-align: justify;"><strong>Hospital stay: </strong>A hospital stay amounts to 9,249 francs ($7,000/$9,200) &#8211; 2011 average of the Swiss Federal Statistics Office.</p>

				<div class="et_pb_toggle_content clearfix"><p style="text-align: justify;">With the dissolution of the Soviet Union in 1991, the health of the Russian population greatly declined (the levels of tuberculosis, cancer, and heart disease are among the highest in industrialised countries). The government has therefore undertaken a succession of reforms since the mid-1990&#8217;s and the health statistics have been improving.</p>
<p style="text-align: justify;">The most important reform was the introduction of a national healthcare system, made compulsory in 1996. It offers healthcare to Russian citizens, as well permanent and temporary residents.</p>
<p style="text-align: justify;">This system covers: primary care, preventative care, emergencies (except for specialised emergencies such as air ambulance), and specialised care in all public medical establishments. However, some services are not offered, such as dental, prosthetics, rehabilitation and convalescence, as well as prescribed medicines. In order for these to be covered you would need to sign up to a private health insurance.</p>
<p style="text-align: justify;"><strong>The structure of health services</strong></p>
<p style="text-align: justify;">In Russia, there are no private surgeries and all practitioners see their patients in either a clinic or a hospital.</p>
<p style="text-align: justify;">The administration of the health services is divided into different levels: federal, regional, and municipal.</p>
<ul>
<li style="text-align: justify;"><strong>Health posts:</strong> run at a municipal level, they offer out-patient treatment: routine check-ups, vaccinations, and the treatment of non-serious accidents.</li>
<li style="text-align: justify;"><strong>Health centres:</strong> with teams of nurses, gynaecologists, paediatricians, and mid-wives; these establishments are usually specialised in maternity and also basic routine surgery. They are also usually run by the local municipality.</li>
<li style="text-align: justify;"><strong>Urban polyclinics: </strong>these are regional establishments, offering a wide-range of services: screening, treatment of chronic conditions, follow-up care. Depending on their size they may even have on-site specialists (cardiologists, oncologists, obstetricians etc).</li>
<li style="text-align: justify;"><strong>Specialised medical hospitals:</strong> usually found in large cities, these establishments offer all medical services and are open 24 hours a day. Federal hospitals represent 4% of the number of beds available within the public sector.</li>

<p style="text-align: justify;"><strong>Quality of healthcare</strong></p>
<p style="text-align: justify;">The Russian healthcare system has seen some difficulties, in particular linked to a lack of funds. Today, healthcare in Russia remains below the standard of other industrialised countries: there is a lack of medical equipment, differing standards of care depending on the medical establishment, shortfalls in primary care etc. The well-reputed hospitals in Moscow or Saint Petersburg do not offer all services (e.g. contagious illness and maternity complications are not treated).</p>
<p style="text-align: justify;">In the public sector, English speaking doctors are few and far between. In order to see an English speak doctor it is advisable to seek treatment at a private clinic, however fees in the private sector can be high.</p>
