Healthcare in Argentina for expats

The National Health Insurance Administration (La Administración Nacional del Seguro de Salud) offers cover for health, pensions/retirement, disability, death (for relatives) and unemployment benefits. Accidents at work and occupational diseases are managed by the superintendencia de riesgos del trabajo (SRT).

Argentina is a federal republic and the health system is decentralized, meaning public health is administered at a municipal level. Primary health care is often independently managed by each city.

There are three sectors of health care in Argentina:

The public sector: Provides free or highly subsidised health services to around 50% of people in Argentina, including both nationals and foreigners. Medical, hospital, dental and palliative care, rehabilitation, prosthetics and medical transport are free of charge. The only charges are for prescriptions, and pharmacies require prescriptions for most medication. There is a 30% cost participation for chronic conditions, and a 60% cost participation for everyday prescriptions. However, long waiting lists mean that a large percentage of Argentina’s population chooses other sectors for their health care.

Social security (obras sociales – OS): Financed by fixed fee contributions from employers and employees, these health insurance schemes are managed by trade unions and are the most common form of health insurance in the country. The schemes are overseen by the national health insurance administration, but health care is outsourced to private providers. Obras sociales schemes must guarantee minimum medical coverage such as preventative, diagnostic, medical and dental treatment, maternity, etc. The patient covers the difference in cost between the fixed fee and the cost of treatment. Additionally, and importantly, OS schemes cannot exclude pre-existing medical conditions.

Private sector: These are like classic private insurance policies which anyone can choose to subscribe to. Around 5% of the population opts for private health insurance. Private insurance must cover the same mandatory benefits as the OS.

To note: General practitioners practice in public hospitals, so there are no public GP offices or centres in Argentina. To consult in a practice, patients must make an appointment with a private physician.

Quality of health care
Health services in Argentina are considered among the best in Latin America. In Buenos Aires and other major cities like Córdoba or Mendoza, hospitals and clinics are of an excellent standard with competent medical staff. However, elsewhere in the country, especially in the poorer north, the quality of care may vary and some facilities may lack medical equipment or staff.

Private health insurance in Argentina
Although the public health system is free and of good quality, waiting lists for appointments and treatment can be long in the public sector. For this reason, many expatriates choose private health insurance.

Argentinian health insurance is very localised, meaning that a local insurance policy may not be valid in other countries and even other cities of Argentina. Therefore, international insurance is usually better suited to an expatriate rather than a local insurance.

If you are travelling in remote areas, it is highly recommended to have health insurance including evacuation, as some regions may be very far from the nearest hospital. In the event of an accident, the evacuation assistance will take care of transportation to a health care facility able to treat you.

It allows you to be treated in the hospital of your choice or with the doctors of your choice, in Argentina or abroad. This is often the best option for people preferring to seek treatment in the private sector, whether in Argentina, their country of origin or another country.

Healthcare in Mexico for expats

Healthcare in Mexico has the reputation to be excellent whilst being affordable compared to other countries. To give an idea, costs are estimated to be 25 to 30% lower than in the United States. This makes Mexico a destination of choice for medical tourism.

However, when living as an expat in Mexico, subscribing to the national healthcare system exclusively might not be sufficient. Although public hospitals are of good standard, they are often overcrowded, especially in big cities. To avoid waiting lists, it is advisable to make use of the private sector. Therefore, an international health insurance for Mexico might be the best solution to help you manage your health care costs.

The Mexican health care system is called IMSS (Instituto mexicano de seguro social). It is mainly for employees in the private sector. Those working in the public sector have their own healthcare system. For the unemployed and retired, healthcare is handled by the Ministry of Health through the “Seguro popular”.

Seguro Popular and IMSS are available to citizens and expatriates who are temporary and permanent residents.

Doctors

Mexicans doctors are usually highly qualified, many having been trained in the United States. Many American doctors also train in Mexico, which highlights the quality of the medical training on offer.

Public hospitals in Mexico

While large cities like Mexico City, Guadalajara and Monterrey have many modern hospitals (and each of these cities boasts at least one internationally acclaimed hospital), in rural zones the available care is limited and there is often a lack of equipment. Many people are therefore sent to hospitals in the cities to be treated for certain conditions. Consequently, these hospitals become saturated and waiting lists can be very long.

Private hospitals and clinics in Mexico

Mexico has many renowned private hospitals and clinics, especially in the larger cities. The medical equipment is modern, they have state of the art technology, and the healthcare personnel are international.

The care in private hospitals is cheaper than in countries such as the United States or certain other Western countries. However it is important to bear in mind that hospitals in Mexico can refuse to accept patients who cannot offer a financial guarantee, even if the patient has medical cover in place. They may ask patients to sign a credit card voucher.

Before patients are discharged all hospitals bills must be paid. This can mean that you may end up staying more nights than necessary in the hospital until payment had been received (either from you personally or your insurance company). It is therefore strongly advised to have a local medical insurance in Mexico or an international health insurance when using private healthcare.

Access to the Mexican health care system for foreigners

Your right to access the Mexican health care system will depend on your residency status.

Non Resident: If you are visiting Mexico for a short period, you will not have access to the Mexican health care system and will have to subscribe to travel insurance issued in your country of residence.

Resident: As a resident in Mexico (temporary or permanent), you apply to the IMSS either on a voluntary or compulsory basis, depending on your employment status:

IMSS compulsory enrolment:

If you are an employee in the private sector, you will be enrolled in the IMSS on a compulsory basis. Employers must enrol their full time employees in the IMSS by law, be they Mexican nationals or foreign residents. The IMSS is financed by contributions made by employers and employees, as well as a contribution from the State.

IMSS voluntary enrolment:

You can enrol in the IMSS on a voluntary basis if you are self-employed, retired or if you don’t work but not if you are a full-time employee.

It’s important to note that the IMSS applies waiting periods or excludes some pre-existing medical conditions to people applying on a voluntary basis. The IMSS can even decline your application.

If you have a pre-existing medical condition, you may want to opt for the Seguro Popular (providing you are eligible) or look for a private insurance that will accept your application (although they might exclude your pre-existing medical conditions).

How to subscribe to the IMSS?

You can begin the application on the IMSS website or at your local IMSS office. All the application process is in Spanish. You can find the list of documents you will have to provide here. The list is in Spanish so here is the translation:

Your marriage certificate, if applicable

Your passport

Your CURP (Clave Única de Registro de Población), which is a personal ID code number allocated to both citizens or residents of Mexico.

Proof of payment of the first premium

Birth certificate

Proof of address

Completed medical questionnaire provided by the IMSS

You must provide the original and a copy of the different documents.

Cost of the IMSS

The price for annual enrolment depends on your age. The annual fee is approximately 7000 pesos (280 pounds or 370 dollars) per person and renewed annually. You can get the current rates from the table on this page.

Whether you are covered with the IMSS or the Seguro Popular, healthcare in Mexico is free and there is no co-payment.

The main downside is that you are not free to choose your own doctor (except in specific cases). You are not covered abroad, in the private sector or in your home country. If you want to be able to see the doctor of your choice, and in order to be reimbursed, an international private health insurance in Mexico is the best option.

International health insurance for expats in Mexico

Most foreign residents who can afford to do so will take a private medical insurance plan in Mexico that gives them access to private doctors, clinics and hospitals. An international health insurance for Mexico is often a better option for expats than a local health plan. It allows you to be treated in the hospital and with the doctor of your choice, not only in Mexico but also abroad (within the zone of cover of your insurance policy).

Moreover, if you leave Mexico and move elsewhere, your international health insurance may be able to follow you, unlike a Mexican health insurance which is only valid locally.

It is important to note that an expat insurance in Mexico will not exempt you from subscribing to the IMSS if it is obligatory for you. The private insurance policy will simply cover you on top of the IMSS.

Yellow fever vaccination certificate is required for travellers over 1 year of age arriving from countries with risk of yellow fever transmission and for travellers having transited more than 12 hours through the airport of a country with risk of yellow fever transmission.

Emergency numbers
All emergencies: 066
Fire brigade: 068
Ambulance: 065

Medicine
The price of common medicines, like aspirin, is much lower than in the UK and it is possible to buy them individually which can work out to be much cheaper if you only need a few instead of a whole box.

Theoretically, many medicines are available only through prescription. In reality, these regulations are not always adhered to for certain medicines like antibiotics or strong pain killers which can be easy to get hold of without first having to see a doctor.

If a person buys medication without a prescription, but would usually have required one, it is their responsibility if there are any problems. Insurance companies will only reimburse medication if they have been prescribed by a doctor.

Healthcare in Brazil for expats

The healthcare system is a great source of pride in Brazil, whose universality and free of charge service is written into the constitution. However, the system is often under-financed and this can impact the quality of the services. Opposite this, the private healthcare system offers high quality care, but often at a high price. It is therefore advisable to subscribe to a private health insurance for Brazil in order to be able to use the private system and be reimbursed for your costs.

The Brazilian public healthcare system is called the « Sistema Unico de Saude » (SUS). It is open to Brazilians as well as foreign residents.

While developing the SUS in the 1980s, Brazil made giants strides in progress when looking to cover its 199 million residents. With the SUS, all care is free: GP consultation, lab tests/diagnostic tests, hospitalisation, surgery and even prescribed medicines. This is the strong point of the Brazilian healthcare system.

Furthermore, on top of the network of public medical establishments available, the SUS also relies heavily on the private sector. Over half of the procedures carried out are carried out by private hospitals, who are then reimbursed by the state. Moreover, many of the SUS doctors also work in the private sector – similarly to in the United Kingdom where many NHS doctors also work in the private sector.

The problem with the SUS is therefore not the quality of the care itself, but the access to the care.

The limits of the Brazilian public healthcare system

The medical and hospital infrastructures are often concentrated within urban areas and these are often insufficient in rural areas (60% of the hospitals are situates in the south or the south-east of the country).

In the provincial cities, hospitals do not necessarily have all the necessary equipment or the specialised doctors to treat certain conditions.

The hospitals in the big cities have much more equipment and specialised consultants; however these hospitals are often over-subscribed as patients are sent there from all over the country to be treated. As such, waiting lists can be very long for certain treatments or for certain operations (many months, even up to two years).

Some hospitals are understaffed or lack air-conditioning (which can be a major problem in a country as hot as Brazil).

Generally, the A&E system tends to be efficient. What can be a problem is access to the treatment for non-urgent conditions.

Due to the difficulties faced by the public healthcare system, the use of the private sector (In Brazil or abroad) has grown tremendously. Today, around 25% of the Brazilian population have private medical insurance. In comparison to other countries offering a public health service, with private medical insurance as an optional additional form of care, this figure is very high.

The private healthcare system

The contrast between the public and private sectors is stark. While the public sector is struggling, the private hospitals and clinics are excelling, offering a quality of care which is equivalent or even superior to European or North American standard. The best hospitals are internationally recognised, with state of the art equipment and highly qualified doctors, often trained in the States.

This has led Brazil to becoming a centre of medical tourism for residents from neighbouring countries, especially Sao Paulo which has the highest concentration of high quality hospitals.

You can cover yourself in 4 different ways:

    Medical plans (planos de saúde)

This is a group medical insurance where the treatment is provided by doctors and medical establishments which are approved by the insurance company. These plans offer good value for money, but are limited to a geographic zone of cover and a network of doctors/medical centres.

    Brazilian private medical insurance

There are around 15 insurance companies authorised to offer their products within Brazil. The difference between these and a « planos de saude » is the fact that there is no restrictive geographic zone of cover or network.

    Self-management plans

Some companies will arrange a contract directly with certain hospitals and doctors in order to manage the health cover of their employees.

    Medical cooperatives

These represent groups of doctors who own a hospital: members would be covered by these doctors/medical establishments.

How much does healthcare cost in Brazil

As the public healthcare system is free the costs below are representative of the costs within the private sector.

As a general rule, private medical costs are quite high in Brazil. They have also greatly increased over the past several years; from 2009, private hospitals operating with standards similar to those found in the States and in Europe increased their costs between 20% and 30%. This means that today Brazil is the country with the most expensive private healthcare system in Latin America.

In regards to a consultation with a GP or a specialist, the fees are set individually and they can vary depending on a number of factors: the urgency and time of the consultation, the qualifications of the doctor, their reputation, and also the area or the place where the surgery is located.

Average costs in 3 large cities:

Brasilia : The cost of a consultation with a GP can vary between 120 R$ and 500 R$ (£25 – £100/ $31 – $130). A consultation alone costs around 150 R$ (£30/ $40). In a private clinic, a routine surgical procedure with no complications can cost between 7,600 R$ and 18000 R$ (£1,500 – £3,700 / $2,000 – $4,700). The cost of a day in hospital is generally around 9,000 R$ (£1,800 /$2,300). (This does not include treatment or tests. Therefore for a cancer, for example, the cost will be much higher).

Rio de Janeiro : A consultation with a GP is often around 200 R$ to 250 R$ (£40– £50 / $50 – $65), with a specialist it would be around 350 R$ – 400 R$ (£70– £80 / $90 – $100).

Sao Paulo : A consultation in a surgery is usually around 210 R$ (£43 / $55) for and GP and 280 R$ (£58/ $72) for a specialist. A consultation with a dentist tends to cost around 150 R$ (£30/ $40). A simple surgical procedure can cost between £2,200 and £4,370 ($2,780 – $5,530) and the cost of a day in hospital can be between £365 and £730 ($460 – $925), not including any treatment or tests.
